Description:TGS is customarily performed under local anesthesia supplemented by a short-acting sedative such as thiopental or propofol for needle insertion. The area that was found to produce the strongest pain inhibition corresponded to the area of the precentral gyrus from which muscle contractions of the painful area could be elicited. Indeed, we have anecdotally noted improved results in patients treated with spinal cord stimulation SCS who participate in behavioral therapies such as biofeedback, stress relaxation training, coping skills, etc. Evaluation by a skilled pain psychologist is invaluable for identifying factors that might influence the effectiveness of treatment such as significant depression, external psychosocial stressors, etc. More recently, stimulation of peripheral branches of the trigeminal nerve such as the supraorbital and infraorbital nerves has shown promise in the management of patients with TNP and PHN whose pain is confined to the territory of the supraorbital or infraorbital nerves [ 3,4 ].
